Patient Forms
New Patients
New patients should have the following:
- Government issued ID such as a driver’s license
- Health insurance cards and any related information
- List of current medications
- Information of Doctors you’d like Dr. Lathia to correspond with
- Medical records that you want to review with the doctor
- For all new patients, the New Patient Packet is available to download and bring them filled out on the day of the appointment
- For Established Patients, appointment specific Patient Forms are available on this page
- HMO insurance holders: we need a referral submitted from your Primary Care Physician (PCP) prior to your appointment
